The first step in enrollment is to verify your school of residence.
Please use the following website by clicking on the button below and entering your address information to see if we are your school of residence.
If we are not, you will require an intra-district permit in order for your child to attend our school.
Intra-district permit applications are available below.
Only the parents or legal guardian may enroll their child in school. Please make sure that the application is filled in completely and that you have met all the requirements at the time of enrollment.
- Age verification - bring any of these documents: birth certificate, baptismal certificate, passport, I-94, or green card
- Immunization record - must be updated
- First Grade only - Physical exam or appointment date with your doctor's name.
- Transfer Report - for Los Angeles Unified School District students from last school attended. Not required for new Kindergarten students.
- Child must be present at the time of registration. With a parent or legal guardian.
- Proof of address - a current water, gas, electric utility bill, rental agreement, mortgage statement, or official government mail. All must be in your name and show your address.
- Emergency Card - must be completely filled out and have at least 3 telephone numbers of people who may pick-up your child in case of an emergency.
- Please bring all of the above documents, along with your picture I.D.
- Enrollment Application - Please fill out all pages.

St. Vincent Meals on Wheels serves anyone in need that lives within our service area, regardless of age, race, illness, disability, religion or ability to pay.
